HOMELESS
SERVICES
HOMELESS FAMILY SERVICES:
Homeless Family Services is comprised of three closely linked outreach
teams, each of which provides unique and specialized outreach services
to homeless families throughout King County. Homeless Family Services
strives to create conditions in which families are able to address their
personal circumstances, obtain the treatment and services that they need,
and to gradually acquire the confidence and stability to reconnect to
their own communities of support and care.

Mental Health treatment services for
Medicaid eligible clients are provided by our team of therapists,
who "extend the front door" of Valley Cities' mental health capacity,
delivering treatment services to homeless children and their parents
throughout King County.
- Families in Shelters: Shelter-based mental
health and chemical dependency services, including psychiatric evaluation,
are provided in family shelters and transitional housing programs
in Seattle and in South King County. VCCC Families in Shelter staff
collaborate with health care advocates from YWCA and with nursing
staff based at Country Doctor Community Health Centers and at Community
Health Centers of King County to provide interdisciplinary health
care services in these settings.

Pathways Home: Pathways Home
provides mobile outreach to homeless families, providing continuity
of services for as long as a family may be homeless and no matter
how many times they might move. The team is composed of nurses, mental
health and chemical dependency professionals, a social worker and
a housing specialist. This is a project jointly operated with the
participation of Puget Sound Neighborhood Health Centers and 45th
Street Clinic.
